Selective Neurectomy
This treatment involves cutting some of the nerves to the calf. This causes the muscle to atrophy and shrink, thus reducing its size. Although minimally invasive, this surgery is irreversible and can cause problems. These include numbness or pain and the possibility of other surrounding muscles becoming larger to compensate.
Liposuction Is the Safest Surgery
Of all the options to reduce the size of the calves, liposuction is the safest. This procedure involves a surgeon making small incisions in the lower leg. He or she will then pass a narrow tube through and permanently suck out the excess fat. The surgeon can then sculpt the calf to produce a more aesthetically pleasing and slimmer shape.

You will be under a local anesthetic, so there are no risks associated with general anesthesia. Also, there are limited negative side effects, and those that may occur are generally short-lived.
